Philosophy of Forms
Forms are a necessary part of a service operation like ours. They are not intended to be administrative hoops but are intended to provide a predictable and orderly proceess through which we can provide the appropirate service and documentation of animal care. Each form in the table below is accompanied by a set of instructions for the new user. If you have suggestions regarding the forms, their content or their style, please let us know so we can respond to your needs.
Husbandry Forms
Special Care Instruction Form
A special care instruction form must be completed in cases where the deviation from standard care is requested. A common example is medicated water provided to animals. The special care instruction form proceeds through an approval process so that communication occurs with supervisory and veterinary staff before changes are implemented. It is advisable to speak directly with animal husbandry managers when initiating special care for any animal. A copy of the form is kept in the Animal Husbandry Data Book in each animal housing room. Cage Request Form
This form should be used when you need extra cages to be delivered to your room on a regular basis. Extra equipment and supplies are placed throughout the facility for your convenience; however, if you need a large number of cages on an ongoing basis, please complete this form so we can supply the appropriate caging to meet your need. Problem Resolution Form
The problem resolution form is provided for investigators to help us document when mistakes occur. Our desire is to find the root cause of each mistake and do everything in our power to prevent the same mistake from happening again. The form also enables us to provide you feedback when we have developed a fix for the problem. We strive always to respond quickly and effectively to problems in order to minimize disruptions to your research. Animal Health and Surgical Records
Procedure Data Sheet
The Procedure Data sheet is used to schedule services with the Veterinary Services Branch. The form should be completed and submitted a minimum of 48 hours in advance of the service. Operatory time is scheduled on a first come first served basis. Communication with the Veterinary Technicians or with a Veterinarian will eliminate roadblocks and delays in performing important procedures. Treatment Request Form
The Veterinary Services Branch of the ARC will perform treatments for investigators on request. Treatment requests must be documented on the Treatment Request Form. Complete this form and submit to the Veterinary Technicians in room EB012D.
Research Animal Clinical Record
The clinical record is the primary means of recording clinical treatment and experimental procedures. The clinical record is an important part of providing compassionate care. The record should be completed in the standard S(Subjective) O (Objective) A (Assesment) P (Plan) Format. Clinical Record Continuation Sheet
The record can be continued on the Clinical Rcord Continuation Sheet.
Anesthetic Recovery Form
This form is used to document monitoring while anesthetic wears off. Continuous monitoring must be provided until extubation. The form remains as part of the permenant clinical record after the procedure is completed.
Postoperative Evaluation Form
This form is used to monitor the first few days following a procedure until an animal has stabilized following surgery. This form is also a part of the permenant clinical record and must remain with the record.
Animal Shipment
Nonstandard Vendor Import Form
This form is used when you want to get animals from colaborators at other institutions. The form applies primarily to mice but may be used for other species. The process of processing, receiving and quarantine is explained in the Nonstandard Vendor Animals Presentation in the training section. Nonstandard Vendor Export Form
This form is used when you want to ship animals to other institutions. Our Institutional Animal Care and Use Committee and the Veterinary Services group have a responsibility to ensure that animals shipped to other institutions are done so with the approval of the receiving animal facility and IACUC. The ARC can arrange and ship the animals for you if you wish. Either way, if you ship the animals or if we ship the animals, we must secure approval before the animals are shipped. This may take some time so planning ahead will help in the process. This form should be submitted to the ARC main office at least one week in advance of the proposed shipment. Animal Transfer Form
In order to make the trasfer of animals between Case animal facilities easier to manage we have developed the Animal Transfer Form. Investigators must either follow the IACUC animal transport guidelines or request that the ARC perform the transfer. The process for transferring animals through the ARC is described in the Animal Transport FAQ.
