Facility Access
Authorized Personnel
- Only authorized personnel are given access to CWRU animal facilities.
- Only personnel listed on an active animal protocol with a completed
personnel health and training form (PHT) will be granted access
to animal facilities
- Access is granted only after completion of training with the
Training and Compliance Coordinator
- Under NO circumstances should unauthorized personnel use another
employees access card.
- All facilities are accessed through a card reader using a valid
CWRU ID card. UH ID cards are no longer valid.
To obtain Facility Access you will need
to :
- Have a valid CWRU ID Card
- Be on an approved animal protocol and have filled out a PHT
form
- Have read and be familiar with the most current version of the
protocol that you are working on
- Have completed training with the ARC Training and Compliance
Coordinator
- Be knowledgeable in the Guidelines for Animal Use in Biomedical
Research Laboratories
- Be enrolled in the University Occupational Health Program (368-2450)
- Complete the Investigator Orientation Module
To
Gain Access to the facilities:
- Report to the IACUC Office in WG77 (368-3815) to confirm that
you have been added to an approved protocol.
- The IACUC Office requests that access be granted through the
UH Security Office and the SOM Facilities Office.
- Activation generally occurs within 24-48 hours.
- Access to the Transgenic Mouse Facility in the Wearn building
is approved by the IACUC coodinator
- The IACUC coordinator issues specific instructions for access
to HU facilities
