PROBLEM RESOLUTION FORM FAQ
Q: What is the purpose of this form?
A: To offer the investigative community an opportunity to communicate
problems, incidents, and other issues that need to be addressed
by the ARC.
Q: Where are these forms located?
A: Forms are available in various locations. They can be downloaded
from this website (Problem
Resolution Form) , picked up in the ARC office in the
Medical School (TB03A), taken from the Animal Room Data Books or
faxed or mailed on request.
Q: What sections should I complete?
A: The investigator should complete the personal information, the
"description of the problem", and the "proposed resolution."
Q: What do I do with the form?
Fax, mail, e-mail or bring the form to the ARC office.
Q: What's the review process?
A: The form is reviewed by the appropriate manager(s) in the areas
of concern. An approved action is recommended and then brought before
the ARC management group for discussion and final approval. If the
issues are complex, and questions still remain, the investigator
may be contacted for additional information and feedback. The final
"approved action" is noted, along with a date of implementation
of any changes, and the completed form is returned to the investigator.
A copy is retained in the ARC investigator files for future reference.
Q: What if the approved action does not meet my needs?
A: Investigators may return the form along with an addendum noting
the remaining problems. The ARC management group will take up the
outstanding issues at the next available management group meeting,
where they will attempt to satisfy the investigator's concerns.
Again, any documentation of approved action will be sent back to
the investigator. The process may continue until the investigator
is fully satisfied with the results.
Q: Why should I bother?
A: The form is designed to provide a complete feedback loop to investigators
in handling problems and concerns. This helps us to document problems
so that the memory of a particular problem does not fade leaving
a recurring problem along the way.
